ADDENDUM NO. 1
ATTENTION: All Bidders and Plan holders

All bidders are required to base their bid upon the information furnished in this addendum; and acknowledge receipt of this addendum as required in the Contract Documents. The following corrections/modifications are hereby made a part of the Contract Documents.
PART 2 – BID PROPOSAL DOCUMENTS
The Bid Proposal Documents are revised as follows:
On page 3.1, bid item #109 name has been corrected to “Remove Storm Drain Pipe”.
On page 3.1, bid item #115 “Pothole Utility”, quantity is revised to 17.
On page 3.1 of the proposal, a new lump sum item “Clearing and Grubbing,” is added.
On page 3.1, 3.2 and 3.3 of the proposal, Bid items 109 through 167 have been renumbered to 110 through 168 due to new bid item added.
In order to incorporate the above noted changes into this project, remove existing pages 3.1, 3.2 and 3.3 of the Bid Proposal Document and insert the attached revised Bid Proposal Documents pages 3.1, 3.2 and 3.3 from Addendum No. 1.
PART 4 - SPECIAL PROVISIONS
The General Special Provisions are revised as follows:
On page SP-19, Section 1-05.14 “Cooperation With Other Contractors”, delete lines 15 to 19 and replace with the following:
• Avista Gas- Potential stormwater conflicts exist with 2” gas line on south side of Wellesley and 6” gas line on north side of Wellesley. Pothole and contact to verify Addendum No. 1 Page 2 City of Spokane Valley Wellesley Ave Preservation Project depths, particularly at approx. STA 6+50 and STA 14+55 for potential stormwater structure and pipe conflicts.
• Lumen has identified shallow communication lines on the south side of Wellesley Ave (Approx. Sta 14+25 to 25+75). Contractor will need to coordinate with Lumen for the relocation and lowering of the communication lines.
• AT&T- At least four (4) 2” conduits exist on the north side of Wellesley. Need to coordinate potholing and potential maintenance sleeving of AT&T conduits with AT&T contractor at approx. STA 6+50 and STA 14+55.
• Fatbeam- contact about buried fiber locations.
• Zayo- contact about buried fiber locations.
On page SP-24, Section 1-07.17 “Utilities and Similar Facilities”, the table containing utility contact information has been updated with the following:

CONTRACT PLANS
The Contract Plans are revised as follows:
Sheet CV1 is revised for a spelling error.
Sheet PH1 has additional potholes.
Sheets DM1, DM2, and DM3 have clearing and grubbing items added, note and keynote clarifications.
Sheets RD1 and RD2 have additional curb elevation information and note revisions.
Sheet DT2 has additional information in detail.
Sheets SD1, SD2, and SD3 have updated invert elevations and pipe slope information.
Sheets CH1 and CH2 have stripe revisions at Tolford Rd and “no parking” signs added.
Sheet ST1 is revised for a spelling error and provides additional information in the table.
Sheets TC1, TC2, TC3 and TC4 have keynote and detail reference corrections, and added a shoulder closure detail.
In order to incorporate the above noted changes into this project, remove sheets CV1, PH1, DM1-DM3, RD1-RD2, DT2, SD1- SD3, CH1-CH2, ST1, and TC1-TC4 of the Contract Plans and insert the attached revised sheets CV1, PH1, DM1-DM3, RD1-RD2, DT2, SD1- SD3, CH1-CH2, ST1, and TC1-TC4 from Addendum No. 1.
